I/we, (hereinafter -“Owner(s)” ) certify that I/we are the owner(s) of an animal, named , for which I/we have sought behavioral counseling from Carolyn Lincoln, DVM (hereinafter -“Lincoln”) and Play To Behave, LLC (hereinafter – “PTB”) for the purpose of receiving advice on decreasing its aggressiveness. In consideration of the services Lincoln and PTB has agreed to provide Owners hereby consent and agree to indemnify and hold harmless Lincoln and PTB, her heirs and assigns, agents and employees ( hereinafter the “indemnified parties” ), from any and all claims, liabilities, demands, suits, causes of actions or proceedings of any kind or nature, losses or damages, including attorneys’ fees and costs of defense, which the indemnified parties may incur arising from the acts of this animal or the Owner(s), whether arising in tort, contract, or any other claims of law or equity.
