This “Lease #8” is entered into as of January 5, 2015 (the “Effective Date”) between Theriac -Warwick, LLC, a Florida limited liability company (“Landlord”), and Southern New England Regional Cancer Center, LLC, a Rhode Island limited liability company (“Tenant”), for the premises (the “Premises”) and improvements thereon (the “Facility”) within the building with a street address of 450 Toll Gate Road, Warwick, Rhode Island 02886 (the “Building”) located on the real property more particularly described on attached Exhibit A (the “Property”), such Premises and Facility used as a radiation or oncology related medical office (as such utilization may be changed pursuant to Section 7.1(a) and collectively, the “Business”).
