These Terms and Conditions (“Terms and Conditions”) form a legal agreement between Montefiore Health System, Inc. (“Montefiore”) and you (“you” and “your”) concerning your access to and use of Montefiore’s telehealth service, Montefiore First. Montefiore First consists of telehealth platforms and infrastructure designed to connect individuals with participating physicians and other health care providers (“Providers”) via real-time interactive audio and video (the “Platform”) for the purpose of participating in a health care evaluation and/or consultation (the “Service”). While the Platform and the Service provided through the Platform are intended to provide benefits such as convenient access to Providers and the Services they provide, there do exist potential risks associated with use of the Platform and the Services, which are highlighted below in the Express Acknowledgements section.
