Current Vaccinations/Veterinarian Information: By signing this agreement, owner verifies their pet(s) is current on Northwood Animal Hospital's ("NAH") core vaccines, such as: Rabies, Distemper, Leptosporosis and Bordetella vaccinations, and that their dog(s) is healthy to the owner’s knowledge. Pet's must be up to date on their annual wellness exam here at NAH.
