This ESO Subscription Agreement (this “Agreement”) is entered into as of the date indicated on the duly executed Quote (“Effective Date”), by and between ESO Solutions, Inc., a Texas corporation having its principal place of business at 11500 Alterra Parkway, Suite 100 Austin, TX 78758, including its controlled subsidiaries, (collectively, “ESO”) and Customer (or the governing or controlling authority thereof), as indicated on the Quote or other ordering document. . This Agreement consists of these terms and conditions (the “General Terms & Conditions”) below, the Quote adopting this Agreement, the Business Associate Agreement, and any Order (as defined below) executed by the parties, including any attachments to such Order.
