This Occupancy Agreement is between Village Ridge Assisted Living and the above named resident. Village Ridge is licensed as an Assisted Living Program by the state of Iowa. This agreement begins on the Move-In date listed above and will be in effect for the duration of the resident’s occupancy at Village Ridge Assisted Living. The resident rents from Village Ridge an assisted living unit, with residence specified in the above listed apartment unit based upon the terms and conditions set forth in this agreement.
