In consideration of the loan that may be granted by virtue of this application subject to the lending policy of Fairchild Cebu Community Multipurpose Cooperative (FCCMPC), I/we hereby waive my/our rights under R.A. No. 1405 and authorize FCCPMPC to verify/validate my pay slip. Furthermore, I/we hereby authorize my present employer __________________________________________ to deduct the monthly amortization due from my salary and remit the same to FCCMPC.
