, (Provider) enters into a Provider Agreement with the State of Alaska, Department of Health & Social Services (DHSS), for the purpose of providing community- based services to adults experiencing serious mental illness (SMI), or youth experiencing severe emotional disturbance (SED) or youth experiencing a substance-use disorder (SUD), who are at risk for institutional or residential care. By entering into this Provider Agreement, the Provider agrees to the following, including all applicable provisions of the following Appendices:
