This agreement (“Agreement”) is effective as described below by and between LabThruput (“Business Associate” or “BA”) and any of the affiliated company(ies) of for which Business Associate is to provide services or functions and which are regulated by Subtitle F of Title II of the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and its implementing regulations (45 CFR Parts 160, 162 and 164) (“the Privacy Rules” and “the Security Rules” or “the HIPAA Rules”), and/or the “nonaffiliated third party” requirements of Title V of the Gramm-Leach-Bliley Act (15 U.S.C. § 6801 et seq.)
