This Master Service Agreement is effective on the date signed on the last page of this document (“Effective Date”) between OPIE Choice LLC, a Florida Limited Liability Corporation, d/b/a Futura International (“Company”) with its principal place of business at 3870 NW 83rd Street Gainesville, FL 32606 and the client (“Client”) named in the OPIE Choice LLC Ordering Document. This Master Service Agreement, with the Ordering Document, and Business Associate Agreement along with any subsequent amendments or Ordering Documents, is referred to as the “Agreement” and will govern Client’s use of the Futura office management software service (the “Futura Service”). Company will perform the following services for Client, as defined in the Ordering Document, and Client will have the following responsibilities in connection with Company, as set forth below.
