THIS SUPPORT AGREEMENT, including all exhibits and schedules, is made and entered into by and between Managed Access to Child Health, Inc., d/b/a Partnership for Child health (“THE PARTNERSHIP” herein) and Daniel Memorial, Inc. (“AGENCY” herein), and is effective as of July 1, 2019. The purpose of this Agreement is to conduct work (“SERVICES” herein) related to Wraparound and INtervention Strategies (WINS) (“PROGRAM” herein)—a program funded by a Criminal Justice, Mental Health, and Substance Abuse Reinvestment Grant.
