This Confidential Separation Agreement and General Release of Claims (“Agreement”) is made by and between _(employee) , on behalf of himself, his agents, representatives, attorneys, assigns, and heirs (collectively, “Mr. Smith”), and the City of Mayberry Police Department (“the Department”).
