This Direct Primary Care Membership Agreement is an agreement (the “Agreement”) between you (“You” or the “Member”) and Sanitas USA, Inc. (“Sanitas” or “We”) for You to enroll as a member of Sanitas’s direct primary care membership program (the “DPC Program”), beginning on the date that You electronically sign this Agreement. We are offering the DPC Program to you on behalf of your primary care provider, Sanitas Medical Center. As a Member, you will be eligible to receive the Included Services defined below.
