This Membership Agreement (this Membership Agreement, as the same may be amended from time to time, this “Agreement”) is between US Fitness Holdings, LLC (“USF”) and the “Member” (as hereinafter defined). This Agreement covers the purchase of a membership in the Crunch Fitness® club located at 12255 Hornsby Lane, Newport News, Virginia 23602 (the “Club”) and any other health or fitness club specified below (together with the Club, each a “club”).
