Furnishing of items and/or fulfillment of services under this purchase order by the vendor named on the front of this form ("vendor," "Contractor" or "you") constitutes a contract between the vendor and the Board of Health for Madison and Dane County, a city-county board of health with the powers and duties set forth in Wis. Stats. sec. 251.04, on behalf of Public Health-Madison and Dane County (“PHMDC”), and your agreement to all applicable terms and conditions as specified in any Request for Bid, Quotation or Proposal and the Conditions of Purchase on this form, all of which are incorporated by reference, and together form this Purchase Order (or “PO”). . Further, this Purchase Order shall constitute the entire agreement and no other terms and conditions in any document, acceptance, or acknowledgment shall be effective or binding unless expressly agreed to in writing by PHMDC.
