I, Parent or Guardian of the named child for a position in the named activity, class, or program, or I (person 18-years or older) hereby give approval for my child’s participation (or my participation) in the activity, class, or program offered by the City of Enterprise, through the Enterprise Parks and Recreation Department. I as- sume all risks and hazards incidental to such participation and do hereby waive, release, absolve, and agree to indemnify and hold harmless the City of Enterprise and Enterprise Parks and Recreation Department, and
