PRE-APPROVED NON-STANDARDIZEDEXPANDED PROFIT SHARING AND MONEY PURCHASE PLAN AND 401(k) PLANADOPTION AGREEMENT #01002 The undersigned Employer hereby adopts the Provider's Pre-Approved Expanded Profit-Sharing or Money Purchase Plan or 401(k) Plan in the form of a non-standardized Plan, as set out in this Adoption Agreement and the Pre-Approved Defined Contribution Plan Document #01 and all completed Addendums, and agrees that the following definitions, elections and terms shall be part of such Plan. Where applicable, certain items have a Default Provision indicated below the item number that will apply if no election is made by the Employer.
