The information is vital so that we can find a suitable match for your animal's disposition, age and health. Use a separate Surrender Agreement for each animal. Along with this agreement, please give us the animal's veterinary records and collars, tags, bowls, toys, a favorite blanket, and a week's supply of current food when you surrender the animal, to help it acclimate to its new surroundings.
