CLINICAL TRIAL AGREEMENT SMLOUVA O PROVEDENÍ KLINICKÉHOHODNOCENÍ This Clinical Trial Agreement (the “Agreement”) is effective as of a date it is published in the Register of Contracts according to the Act No. 340/2015 Coll., on Special Conditions of Effectiveness of Certain Contracts, Publishing of Contracts and Register of Contracts (Act on Register of Contracts) (the “Effective Date”) and is entered into by and between Astellas Pharma Europe B.V., located at Sylviusweg 62, 2333 BE Leiden, the Netherlands (“Sponsor”); INC Research, UK Ltd., located at Riverview, The Meadows Business Park, Station Approach, Blackwater, Camberley, Surrey, GU17 9AB, UK (“INC Research”); and Kroměřížská nemocnice a.s., located at Havlickova 660/69, 767 01 Kromeriz, Czech Republic (“Institution”).
