I, as a parent/guardian of , agree to allow Renovation Church and its volunteers to provide events and activities for my minor. The undersigned does consent to let above specified child participate in all activities/events being organized by Renovation Church. I understand that there are inherent risks involved in any ministry or athletic event, and I hereby release Renovation Church, its pastors, trustees, officers, employees and any volunteers from any and all liability for injury, loss, or damage to person or property that may occur during the course of my child's involvement, which includes negligence caused by the child's own conduct. In the event that he/she is injured and requires the attention of a doctor, I consent to any reasonable medical treatment as deemed necessary by a licensed physician.
