This Nondisclosure Agreement (this “Agreement”) is entered into between Community First Health Plans, Inc (the CFHP”) and Company Name as of , 2017 (the “Effective Date”), to protect the confidentiality of certain confidential information of each Party to be disclosed to the other Party under this Agreement solely for use in evaluating or pursuing a business relationship (the “Permitted Use”).
