d/b/a Christian Investment Advisors a/k/a CIS Wealth Management (herein referred to as “Advisor”), and (herein referred to as “Client”), to provide Client with financial planning services as indicated in Item 1 of this Agreement. This Agreement becomes effective on the date in which the Advisor receives the signed Agreement. The terms and conditions of this Agreement are as follows:
