is entering into a rental agreement with the Italian Benevolent Society, for the use of our grounds on . The Renting party has read and agrees to all maters disclosed in the rental agreement, and accepts all responsibilities for any and all property damage or personal injury which may occur. If you intend to cook in your booth, proper fire extinguishing equipment must be on hand at all times. In addition, propane tanks must be equipped with OPD valves. Please list your menu, or items you wish to sell and return this form with full payment as soon as possible. The Italian Benevolent Society Inc. reserves the right to approve or disapprove of any and all menu items. Furthermore, the agreed upon menu will not be allowed to change. Only one vendor of any one type or style of food will be allowed at the picnic as to eliminate any duplication.
