This Data Use Agreement (“Agreement”) is entered into and made effective on the date last signed below (the “Effective Date”) between Merchant Medicine, LLC, a Minnesota limited liability company (“Data Recipient”) and ____[legal entity] , LLC, a [state] Limited Liability Company (“Covered Entity”). This Agreement sets forth the terms and conditions pursuant to which any De-identified Data Set Information will be handled by Data Recipient, its Affiliated Companies, and permitted third parties during the term of the Agreement and after its termination. For the purposes of this Agreement, “Affiliated Companies” shall mean any entity which is under common control with the Data Recipient, including Urgent Care Partners, LLC and Health System Advisors, LLC, two Minnesota limited liability companies.
