THIS ADDENDUM supplements and is made a part of the Synergistic Office Solutions, Inc.’s Software License and Software Support Agreement. This Business Associate Agreement establishes specific legal obligations regarding the terms of the relationship between Synergistic Office Solutions, Inc. and _ ("Licensee"), and is intended to comply with Standards for Privacy and Security of Protected Health Information (“PHI”) published by the Secretary of the U.S. Department of Health and Human Services (“HHS”) to amend 45 CFR § 160 and § 164 (the "Privacy and Security Regulations") under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and with the Health Information Technology for Economic and Clinical Health Act (the “HITECH Act”), signed into law as part of the American Recovery and Reinvestment Act (“ARRA”).
