Mid-Hudson Cablevision, Inc. has been rebranded to Mid-Hudson Fiber. These Terms and Conditions (“Terms and Conditions”), and any Service Orders (as described in Section 2 below) together constitute the “Master Service Agreement” by and between the undersigned customer (the “Customer”) and Mid-Hudson Cablevision Holdings, Inc., (MH) a New York corporation having its principal offices at Catskill, New York (together with the Customer, the “Parties” or each individually a “Party”) for the services specified on the Service Order(s) (“Services”).
