This ENGAGEMENT AGREEMENT is made effective as of DATE, by and between FULL SCHOOL NAME AND ADDRESS, and Heads Up Educational Consulting, LLC, a Michigan Limited Liability Company (620-A Woodmere, Traverse City, MI 49686) whose president is Patrick F. Bassett and CEO is Richard F. Odell. In this Agreement, the party who is contracting to receive services shall be referred to as "SHORT NAME," and the party who will be providing the services shall be referred to as "Heads Up."
