LAB, SS, EZ1 Advanced XL Instrument, Reagents, Supplies and Service Effective upon signature by both parties, it is mutually agreed that the Contract referenced in this amendment is amended as follows: 1. Pursuant to the Uniform Terms and Conditions, Provision Five (5), contract Changes, Section 5.1, the Contract is hereby revised with the following: 1.1 Provision Three (3), Contract Extensions (5 Year Maximum), of the Special Terms and Conditions is revised and replaced with following: 3. Contract Extensions (5 Year Maximum) The Contract term is for a one (1) year period subject to additional successive periods of twelve (12) months per extension with a maximum aggregate including all extensions not to exceed five (5) years.
