I, the undersigned, hereby agree to play with the above named team during the season or until I am given my release, in writing, by said team and it is recorded with the office of the Onalaska Park and Recreation Department. I acknowledge that I have read and understand the rules and requirements governing my participation in the above named league and agree to abide by them. I certify that I am in normal health, capable of participating safely in the league with which I am registered. I acknowledge and understand that there are certain risks involved in participating and that accidents and injuries may occur in the sport, and that this sport requires proper training and proper physical conditioning. Knowing the risks and conditions required for my sport nevertheless, I hereby agree to save harmless the City of Onalaska from and against all liability, damage or claims of whatever nature arising from injury to myself as a result of my participation in this league.
