WOOLWICH SLEDGE HOCKEY EQUIPMENT AGREEMENTEquipment Agreement • August 17th, 2018
The Following Equipment has been given to me for my use while I am a fully registered paid up member of WSH(Woolwich Sledge Hockey). I am providing a post dated cheque for $200 dated the 15th day of October in respect of the value of equipment I have been given to use. When I return the equipment my post dated cheque will be returned to me. In the event I do not return to the club the following season I will be responsible to return all equipment that I have received by the 15th day of October so someone else may benefit from its use. In the event that I do not return the equipment or re-register by the 15th of October of the current year my cheque will be cashed and the equipment will be picked up by a designate of the WSH club.
