This Amendment is made effective the day of , 20 , by and between EmblemHealth companies Group Health Incorporated (“GHI”), Health Insurance Plan of Greater New York (“HIP”) and HIP Insurance Company of New York (“HIPIC”) with their place of business at 55 Water Street, New York, New York 10041 (hereinafter referred to collectively as "Covered Entity"), and with its principal place of business located at hereinafter referred to as "Business Associate", (individually, a "Party" and collectively, the "Parties"). This Amendment modifies any Selling Agent Agreement and/or General Agent Agreement and/or Managing Administrator Agreement (collectively the “Agreement”) between Covered Entity and Business Associate as set forth below. This Amendment will also serve as Appendix A of the GHI Selling Agent Agreement, GHI General Agent Agreement and/or GHI Managing Administrator Agreement and will replace any prior versions of such Appendix A.
