THIS AGREEMENT (this “Agreement”) by and between Change Healthcare Solutions, LLC (“CHC”) and the party registered to use the CHC Services (“Provider”) consists of the terms and conditions set forth below (the “General Terms and Conditions”) and pricing and other terms applicable to the CHC Services selected by Provider.
