To ensure the effective implementation of the Community Connectors program throughout the state, applicants are required to partner with eligible school districts to provide youth mentoring programs as defined by the grant requirements. Each applicant must identify the eligible district(s) and grade levels of students to be served through the program in question 5 of the grant application.
