THIRD AMENDMENT TO SERVICES AGREEMENTServices Agreement • May 28th, 2021
WHEREAS County and Contractor executed a Services Agreement (the “Original Agreement”) dated as of May 8, 2018, whereby Contractor agreed to provide child welfare services, in the form of Child and Family Team Meeting Facilitation, delivered in the context of a child and family-centered, child and family team that shares responsibility to assess, plan, intervene, monitor and refine services over time, pursuant to Welfare and Institutions Code Section §16501 and §16501.1(c) and (d) for Amador County Social Services Department Child Protective Services, upon the terms and conditions set forth in the Original Agreement. The Original Agreement is referred to herein as the “Agreement.”
