In consideration of participating in the Special Olympics Florida Sunshine Bowl (event), I represent that I am 18 years of age or older, that I understand the nature of the Special Olympics Florida Sunshine Bowl and that I am qualified, in good health, and in proper physical condition to participate in the Special Olympics Florida Sunshine Bowl. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the Special Olympics Florida Sunshine Bowl.
