The undersigned has executed a User Agreement (the “User Agreement”) providing for its participation in the Trade Information Warehouse (the “System”) operated by DTCC Deriv/SERV LLC and DTCC Derivatives Repository Plc (each, a “Company” and collectively, the “Companies”). By executing that User Agreement, the undersigned, each User, as defined in the User Agreement, and each Company agreed to be bound by the Operating Procedures of each respective Company, the Trade Information Warehouse (collectively, the “Operating Procedures”) and the Applicable Publications (as defined in the Operating Procedures), as each may be amended, supplemented or modified from time to time, beginning as of the date entered below (the “Effective Date”).
