PARENT FEE AGREEMENT Name of child Days enrolled Fee Amount Fees are paid weekly. Fees are due each Monday. Services to be provided: Includes family discount Placement in Little Acorns Learning Center will be provided on the above days. Payments are to be made each Monday. Failure to pay the account in full will result in a $5/day late payment fee. Failure to pay the account in full by Friday may result in the account being placed on the waiting list for care until paid in full. Families who are eligible for CCW funding will be responsible for full tuition payments if ineligibility is determined during enrollment and are responsible for a weekly $5/child add-on fee to the copay. All familes are to provide a morning and afternoon snack. LALC will provide lunch. Families of children who are not potty trained, must provide diapers, wipes, and creams and a change of clothing.
