The Health and Human Services Commission (“HHSC” or “System Agency”) and County Judge of Harris County (“Grantee”) which are collectively referred to herein as the “Parties” to the Harris County Jail Diversion Contract, effective on September 1, 2015, and denominated Contract No. 2016-048312-001C (the “Contract”), as amended, now desire to further amend the Contract. This Amendment will be denominated as Contract No. 2016-048312-001D.
