By signing this Authorization To Add Charges To Utility Bill (Authorization), you consent to allow the electric or gas utility checked below (Utility) to include monthly Financing Charges for repayment of your Eligible Agreement in your monthly utility bill from the Utility for the Service Address (Bill), and you agree to pay those charges on the terms described below.
