This Affiliation Agreement (“Agreement”) is made and entered into as of , 2018 (the “Effective Date”) by and between The School Board of Sarasota County, Florida (“School”) and Sarasota Facility Operations LLC d/b/a Consulate Health Care of Sarasota (“Care Center”), for the purpose of providing clinical experience to students enrolled in the School’s Health Science Program (“Program”).
