THIS AMENDMENT is made as of April 1, 2017 (“Effective Date”), by United Behavioral Health of New York, I.P.A. on behalf of itself and as successor in interest (in whole or in part) to United Behavioral Health, AmeriChoice of New York, Inc., Managed Health Systems of New York, Inc., The Travelers Insurance Company and The MetraHealth Insurance Company; and (b) Oxford Health Plans (NY), Inc. (collectively, “UBHIPA”).
