Permanent ownership of this cat (Name: UAF ID # ) is contingent upon your compliance with this Spay/Neuter Agreement. Proof of spay/neuter must be received by United Animal Friends (UAF) within fourteen (14) days after the surgery. If, for any reason, this agreement cannot be honored, UAF must be provided with a statement from the veterinarian indicating when the procedure is scheduled.
