Given the extraordinary circumstances in which we have returned to school this fall, it is critical that health and safety protocols, district guidelines and expectations are clear. After reviewing this document, and with your child (age appropriately), you must acknowledge and agree to the following contents of this form in order for your child to return to in-person learning. Please read and initial each section, sign and date the document, and send it back to your child’s teacher before the first day of in-person instruction.
