_________________________________________, (Provider) enters into a Provider Agreement with the State of Alaska, Department of Health (DOH) Division of Behavioral Health (DBH) for the purpose of providing behavioral health crisis services to individuals (all ages) who are uninsured and/or ineligible for Medicaid. These services include mobile outreach crisis response, peer-based crisis services, 23-hour crisis observation, crisis residential and stabilization services, and continuity of care. By entering into this Provider Agreement, the Provider agrees to the following, including all applicable provisions of the following Appendices:
