We are pleased to provide your child with a school-owned instrument for his/her use during the 2018-19 school year. Families must pay a yearly usage fee to cover the cost of maintenance and repair resulting from regular use. A detailed explanation of this fee is available on the district website at www.hempfieldsd.org/Page/397. If you qualify for economic hardship status or if you are concerned about your ability to pay this fee, please complete the Waiver Request portion of this form. This completed form and/or payment should be returned in the envelope provided.
