This Partner Agency Privacy and Data Sharing Agreement (the “Agreement”) is entered into by and between the County and (the “Partner Agency,” or the “Agency”). This Agreement governs access to and use of the SCC HMIS by the Partner Agency. The Effective Date of this Agreement shall be the date of the last signature of a Party to this Agreement.
