The person signing this agreement is responsible for supervision of all people that will have access to the tree. Submitting this request does not automatically grant approval for access to the tree. You will be notified by e-mail when your request has been approved or denied. The Authority reserves the right to refuse access at any time and for any reason (staffing is limited and may not be available). By signing this, you agree to hold harmless the Authority and their respective directors and employees from and against any and all suit claims, losses, injuries, penalties, demands, expenses, or judgments arising from or in connection with access to the tree. I have read the rules and regulations for access to the tree and will abide by them.
