To the extent that North Broward Hospital District d/b/a Broward Health (“Covered Entity”) discloses Protected Health Information to , (“Business Associate”) (Covered Entity and Business Associate may each be referred to as a “party” and together as “parties) in connection with services or products provided to Covered Entity, or as otherwise required by the Health Insurance Portability and Accountability Act of 1996, as amended, (“HIPAA”), Covered Entity and Business Associate agree to the following terms and conditions, which are intended to comply with HIPAA, the Health Information Technology for Economic and Clinical Health Act of 2009 (the “HITECH Act”), and the Florida Information Protection Act (section 501.171, Florida Statutes).
