This Non-Exclusive Network Access and Use Agreement (“Agreement”), is made and entered into as of the day of , (“Effective Date”), by and between the Utah Telecommunication Open Infrastructure Agency, an interlocal cooperative agency organized under the Interlocal Cooperation Act, Title 11, Chapter 13, Utah Code Annotated 1953, as amended (“UTOPIA”), and (“Service Provider”).
