Service and Assistance Animal Guidelines and Agreement FormOctober 15th, 2020
This document is a supplement to the existing “Guidelines Regarding Service Animals” issued in December 2006 and updated in March 2011. The document can be found at the following link, http://www2.umf.maine.edu/accessibility/service-animal-guidelines-3/. This agreement form applies to a student who wishes to live in the University housing. The Disability Support Services staff is responsible for implementing this agreement form and for assisting students with disabilities to document their specific need for an accommodation. The process for making formal housing accommodation requests starts with a student submitting documentation from their doctor/medical professional. The documentation should include the following:
